Medical Social Worker (MSW/LCSW)-Clinical Resource Management (Per Diem, Day Shift)

A master prepared social worker responsible for complex psychosocial interventions and facilitating the plan of care through discharge planning and resource utilization.  Proactively collaborates with RN Case Managers, Nursing Staff, Medical Staff, and ancillary departments integrating Clinical Resource Management's goals and services with other aspects of the patient care process.  The social worker and the healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values.
Education: Masters degree from a program accredited by the Council on Social Work Education.  LCSW preferred.  ACM (Accredited Case Manager) preferred. Spanish bilingual preferred.
 Experience:  1-2 years prior experience in licensed healthcare agency and/or acute care medical center preferred.  Will consider candidates who have completed internships in an acute care setting.
Knowledge: Strong clinical skills in biopsychosocial assessment, crisis intervention, counseling, interdisciplinary collaboration, and community resources. Knowledge of discharge planning and resource utilization strategies. Knowledge of child, elder, and dependent adult abuse and domestic violence reporting requirements and other significant regulations affecting clinical social work practice (conservatorships, Tarasoff, Lanterman Petris Short act).  Knowledge of human growth and development necessary to provide age-specific assessment and interventions with the following populations-infants, adolescents, adults, geriatrics.

Knowledge of substance use and dependence assessment and interventions strategies.  Knowledge and understanding of human behavior and communication, case management, and the influence of cultural and spiritual values in social work practice.

Knowledge of the utilization management process which includes level of care, case facilitation, appropriate length of stay, and resource coordination.

Special Skills/Equipment:  High level interpersonal skills to communicate effectively, works collaboratively as a team and able to resolve problems through timely and constructive interactions.  Strong organizational skills to prioritize and manage an appropriate caseload of patients with competing demands.  Demonstrates attention to detail, high level of personal accountability and professional integrity, and strong critical thinking skills.  Computer skills--specifically Outlook, Word, Internet, and ECIN.
