Case Manager

As a Case Manager you will provide screening, orientation, assessment, and counseling services to patients. Perform individual and group counseling to patients and families. Coordinate intakes and contribute to the treatment process.

Responsibilities Include:
  • Application / Assessment / Intake
    • Interviews applicants requesting admission and completes initial assessment and treatment plan. Answers calls from individuals seeking information and assistance related to HIV and chemical dependency issues. Offers basic information regarding HIV, chemical dependency and service availability. Provides immediate crisis intervention, as needed. Completes pre-screening form and establishes intake appointments.
    • Completes Management Assessment Intake Form to conduct a comprehensive diagnostic assessment of individual’s biopsychoscial situation and identify needs. Documents demographic, psychological, medical, financial information and individual’s risk factors, disease status, and addiction and treatment history.
    • Performs assessment and identifies risk factors in order to establish suspected mode of transmission, identify high-risk behaviors and develop a risk reduction plan for patient.
    • Provides applicant with an overview of the nature, goals and services of treatment. Explains system of ongoing care plan.
    • Based on information collected in the Case Management Assessment Intake Form, identifies problem areas to be treated and lists them in appropriate section of patient’s care plan. Indicates applicant’s strengths and support systems.
    • Provides referrals for those not accepted into treatment.
  • Treatment Planning
    • Reviews information from Assessment / Intake package and presents information to assigned interdisciplinary team at case conference.
    • Based upon Biopsychoscial Summary and problems identified during Application/Assessment/Intake process, interviews patient and coordinates efforts of Interdisciplinary Treatment Team in order to develop a Care Plan that clearly states problems to be treated, short and long-term goals, specific activities to be undertaken, who is responsible for completing each activity, and timeframes for completion.
    • Based upon information gathered during Intake process and Biopsychosocial Addendum interview, writes a comprehensive diagnostic assessment that provides in-depth history of patient’s illness progression, and a review of their current needs, strengths, and support systems.
    • Monitors patient’s progress in treatment and continually reviews Treatment Plan with patient to ensure continued relevance.
    • Completes required treatment reviews.
    • Updates treatment plans as necessary and appropriate and obtains all necessary signatures and approvals.
    • Initiates discharge planning process. Finalizes aftercare plan and makes all necessary referrals. At the time of actual release, completes the Discharge Summary.
  • Case Management
    • Functions as a case manager with overall responsibility for providing oversight of treatment being provided, including establishment and continued review of treatment goals; coordinating referrals and engagement in ancillary support services; and resolving any problems experienced by patient.
    • Serves as primary contact with insurance carriers and managed care companies; ensures patients are appropriately certified. Assists with obtaining entitlements.
    • Functions as a patient advocate to assist in attaining necessary services in an appropriate setting and within reasonable time frames.
    • Works with family and/or significant others to develop support systems necessary to further treatment goals.
    • Monitors attendance, provides appointment reminders, and follows up with missed visits, as required.
    • Schedules clients for appointments and activities and/or follow-up care in accordance with departmental policy and within expected time frames.
    • Provides patients with referrals to appropriate services in accordance with identified needs and assists patient with accessing desired services.
    • Initiates and facilitates movement of patients between levels of care and provides necessary orientation, education, assistance and support.
  • Counseling
    • Establishes therapeutic rapport with each patient assigned to caseload.
    • Provides basic orientation to new patients regarding treatment, treatment planning, and levels of care.
    • Provides individual counseling to patients to assist them with addressing problems and exploring potential solutions, and setting realistic goals and developing implementation strategies. This includes focus on “Stages of Behavioral Change” and utilizing motivational counseling techniques to implement specific goals.
    • Provides crisis intervention and support.
    • Engages family members and significant others in the treatment process via individual, collateral, or group sessions
  • Patient Education
    • Assists with identification of patient needs, development of appropriate educational activities, and material selection.
    • Provides individual education and assistance to patients regarding a wide range of issues and in accordance with identified needs and learning abilities.
    • Performs and/or assists with outreach activities designed to educate the general public regarding HIV, availability of treatment services, and to further program goals.
    • Assists patients with developing awareness of resources and options open to them.
  • Documentation
    • Records all significant occurrences and events that impact upon the patient and his/her program treatment.
    • Documents all activities, services, and referrals provided in accordance with departmental policies and procedures as well as patient’s response and ultimate outcome.
    • Completes all notes in accordance with policy, within specified timeframes, and in a comprehensive and professional manner that relates back to treatment goals.
    • Obtains required consents and/or releases and ensures completion in accordance with applicable policies. Follows up to ensure requested information was forwarded appropriately, as needed.
    • Provides documentation required by third party payers and maintains ongoing, open communications.
    • Completes departmental statistical reporting forms, as required.
    • Completes required forms from funding sources.
  • Performance Improvement
    • Participates in peer chart reviews and mentoring programs meant to ensure staff competencies.
    • Completes and/or participates in assigned performance improvement activities and attends PI meetings and training sessions as appropriate.
    • Participates in unit specific or department wide PI projects, as requested.
  • Professional Development
    • Knowledgeable about HIV, addictions, various treatment alternatives, and wide variety of HIV and addiction/recovery related topics.
    • Knowledgeable about treatment resources available at hospital in the community and refers patients in accordance with identified needs.
    • Attends SIUH mandated in-service training sessions.
    • Participates in planning and development of specific in-service training sessions and professional departmental activities. Provides specific in-service training as assigned.
    • Participates in professional organizations and community activities related to HIV issues.
    • Attends community based HIV-specific training sessions, seminars and conferences to further improve professional skills and/or is engaged in a formal program of study related to furthering professional knowledge and credentials.
    • Obtains specific credentials within expected time frames or fulfill re-credentialing requirements as per specific licensure.
    • Attends and participates in staff meetings as instructed.
  • May assist with patient transportation between program units, within hospital, to outside activities and/or to outside referral sources.
  • Assists in planning and implementation of various community activities and encourages patient participation.
  • May assist with community relations outside specific program area/hospital.
  • May be required to make home visits, as necessary and/or appropriate.
  • Bachelor’s Degree in Psychology, Social Work, Sociology, Public Health or related field, required. Masters Degree, preferred.
  • Minimum of one (1) year work experience in HIV or substance abuse services, required.
