Sutter Care at Home is the home health care and hospice affiliate of Sutter Health, one of the nation's largest not-for-profit health care systems.

Under the supervision of the Referral Center Supervisor, the Hospital Liaison, in cooperation with Primary Care Physician, hospitalist and hospital staff, facilitates the Hospice admission process. The Hospital Liaison is responsible for attending hospital rounds, facilitating orders, assessing patient needs, including level of care, DME, comfort medications and transportation. The Hospital Liaison enters referral into Electronic Medical Record and completes referral process and admits patient as appropriate. The Hospital Liaison forwards hospital documents to Hospice and communicates with ospice and Hospital Staff. The Hospital Liaison provides education to hospital staff and physicians.
