Ministry Home Care is looking to add a Physical Therapist to our Door County, WI team! This position is Full-Time and will work in the Door County and surrounding areas providing rehab care to patients in their homes.

 

This is an opportunity to help patients achieve the best outcomes by providing physical therapy services in the comfort of their own home.  This Home Health division for Door County offers flexibility in scheduling - allowing excellent flexibility for those with busy lives.  No weekends or holidays scheduled.  Even if you have never thought about working in home health before, we can offer you an opportunity to do a ride-along visit with an experienced therapist, who can show the benefits of working in this area. We will show you that we truly treat each person as an individual and we make our patient's lives more comfortable by caring for them in their own home.  

 

Rehab professionals at Ministry Health Care put patients first and work with a multi-disciplinary team to provide comprehensive rehab services.  Rehab professionals assist patients of all ages with varying functional limitations. As a Ministry employee you are surrounded by a supportive network of highly qualified and licensed professionals working toward a common goal, the patient.

 

In this position, the incumbent will:

• Travel to patient homes to implement and collaborate with the plan of care.
• Understand and assure regulatory compliance issues.
• Demonstrate efficient and effective use of available resources.
• Serve as a role model and practice resource for agency staff.
• Monitor and evaluates patient care outcomes.
• Demonstrate expertise in the delivery of patient care.

MON

• Bachelor's degree in physical therapy from an accredited university
• Minimum one year clinical experience preferred
• Licensed as a physical therapist by the State of Wisconsin
• CPR certification
• Must have the ability to connect to the internet with company issued laptop from home using dialup or hi speed connection (DSL, cable, or satellite).
• Valid driver's license, use of car and proof of auto insurance required
