Case Manager - RN / Per Diem / Days / 8 Hour / Care Management - Mission Viejo Campus

Our Care Management department is looking for a Case Manager to join their team.
Mission Hospital’s Care Management approach is unique. They are a cross functional team where our Case Managers – both RNS and Social Workers create a strong partnership as they develop a care plan for each patient. It is critical to have experience in an acute care setting managing patients with all levels of acuity and working on a blended team.
The CM role is to assess each patient and establish the appropriate plan of care in order to achieve a quality outcome for the patient at a cost effective manner. Patient population includes all ages from Neonates to Geriatrics.
Duties would include:
• Carries out activities related to utilization management, determination of post-acute clinical care needs (discharge planning) and level of care, care coordination and referral to other levels of care.
• Works with physician, social worker, outcomes managers, and interdisciplinary team to facilitate clinical pathways and achievement of desired treatment outcomes.
• Promotes interdisciplinary collaboration and champion’s service excellence.
• Works collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care utilizing decision support criteria.
This opportunity is a Per Diem position, working Days, 8 hour shifts and the minimum commitment will be pick up 2 weekend shifts a month.
Required Education:
• Bachelor’s Degree in Nursing - BSN
Required Experience:
• Minimum 5 years of clinical experience in acute care facility and 6 months discharge planning or utilization review.
• Knowledge/expertise in Interqual or Millennium Guidelines.
Required License / Certification:
• Current Healthcare provider BLS (American Heart Association)
• Current RN license
• Ideal candidate will have Certification in Utilization Review, Care Management, or Clinical Nursing area.
