Care Manager-Registered Nurse Health Home

In the role of Care Manager, you will work with members, family, caregivers and providers to identify and assess member needs. You will coordinate care across acute, long term care and community-based settings to target reduction of medical complications through identified high risk triggers, compliance with medical appointments and community based care. Further, you will promote education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes and function as team leader to interdisciplinary team of clinical and non-clinical staff.
Job Responsibilities Include:
- Collaborates with physician and practice staff in identifying members eligible for care management, utilizing established Care Management criteria.
- Conducts initial and periodic assessments of managed care population, and visits members in their home environment, as required.
- Formulates and implements a care management plan that addresses members identified needs by assessing member/family needs, issues, resources and care goals.
- Utilizes Telehealth to enhance members' access to care and to improve clinical outcomes.
- Identifies and effectively utilizes community resources to help meet the needs of member/families.
- Promotes member self-management and empowers member/families to achieve maximum levels of wellness and independence.
- Encourages members to identify, develop and utilize their support systems.
- Communicates with members, family, providers and plan staff to coordinate patient care.
- Travels to various hospitals and provider facilities to conduct on-site utilization reviews and provider orientation/education.
- Completes all documentation in accordance with policies and procedures of the Health System, State and Federal regulations.
- Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN Degree within five (5) years of employment date.
- Currently licensed and registered as a professional nurse in the State of New York.
- Case Manager Certification (CCM), preferred.
