As a Home Health Nurse, you will empower patients and families to make their own decisions and focus on increasing patient quality of life. This is accomplished through providing nursing care to patients in their residence.
As a Visit Nurse you will conduct visits for assigned patients and their families following the plan of care established by the Care Manager; manage assigned caseload on a daily basis. In the absence of the Care Manager, demonstrated the ability to admit the patient completing designated activities.
Casual Position

Graduate of accredited school of nursing

Minimum 2 years acute care/medical surgical nursing experience preferred

Minimum of 1 year home health experience preferred

Current Wisconsin nursing license or eligible to obtain

CPR certification or the ability to gain certification

Current drivers license, proof of auto insurance, and vehicle availability
