Affinity Medical Group (Family Practice) in Chilton has an excellent opportunity for a 36 hrs/week, salaried Behavioral Health Care Coordinator (LCSW). Hours will primarily be Monday through Friday from 8 am to 5 pm, along with a call rotation.  Primary site is in Chilton, but schedule will also include outreach to Affinity Medical Group/Kiel.

 

As part of the clinic medical home treatment team, this role identifies, triages and provides therapy for patients with medical and behavioral health needs within the primary care setting. Provides skill training through psycho-education and patient education strategies and will develop specific behavioral treatment/change plans for patients and behavioral health protocols for target populations. The Behavioral Health Care Coordinator will also assist patients in coordinating care and connecting with resources.

 

The Behavioral Health Care Coordinator must also be highly committed to team construct and assist the manager with team development and promoting team cohesiveness.

  

§  Masters Degree in Social Work (MSW, MSSW); current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Wisconsin required; billable as a Medicare B provider required.

§  Minimum of four years experience in a behavioral health or clinical social services setting required. Previous experience in a medical setting or primary care clinic preferred.

§  Excellent working knowledge of behavioral medicine and evidence-based treatment for medical and mental health conditions. Knowledge of psychopharmacology preferred.

§  Ability to design and implement clinical protocols for treatment of selected mental and behavioral health conditions.

§  Ability to design effective psycho-education curricula and lead effective psycho-education classes.

§  Ability to train medical providers and other behavioral health providers in the behavioral health aspects of the medical home model.

§  Ability to effectively represent Affinity Health System’s interests in the community and maintain effective working relationships among co-workers, public, private and professional groups.

§  Working knowledge of community-based resources and financial and legal resources. Ability to lead in the areas of team development and conflict resolution.
