St. Elizabeth Hospital in Appleton is looking for a Social Worker to join their Case Management Department!
This is a part-time opportunity (10:00am to 6:00pm)

As a Social Worker, you will be expected to:
- Provide consultation and counseling to patients and their families.
- Provide social services assessment and establish a written plan of care.
- Maintain cooperative relationship with community agencies and programs.
- Remain knowledgeable and comply with medicare and private insurance reimbursement guidelines and procedures.
- Participate in appropriate multidisciplinary and/or specialty teams.
- Consult and/or research difficult decisions with physicians, hospital staff, or community agencies as appropriate.
- Participate in discharge planning process and incorporate utilization of appropriate resources. Follow up with patients after discharge to ensure that plan of care is appropriate.
- Demonstrate critical thinking skills in the decision making process. Is able to prioritize actions to meet changing needs of the patient and nursing unit. Respond according to policy/standards for patient emergency situations.
- Prepare and maintain all documentation as required in accordance with department, facility, and professional standards.
- Provide consultation and coverage to other units as requested
In this role, you will cover our Emergency Department and Palliative Care Department. Our Palliative Care Department is set to take off this summer, so you will be able to be a part of a new, growing department at St. Elizabeth Hospital. You will also be spearheading a new project in the Emergency Department, focusing on chronic care. You will have to be able to effectively balance your time between the continuous Palliative Care and Emergency Department referrrals.
This position will be scheduled 10:00am to 6:00pm Monday through Friday (variable days of the week), 24 hrs/wk (part-time, benefit-eligible), with every other Saturday included (also scheduled 10:00am to 6:00pm). Job Requirements:
- Graduate from a Master's prepared Social Work program, MSW and current WI licensure as a Social Worker required.

Experience in a hospital or long term care setting preferred.

Knowledge of state and federal regulations pertaining to skilled nursing facilities preferred.

Basic Life Support (BLS) certification required.
