Home Health Social Worker MSW/BSW - Part-time position.

At Ministry Home Care we are focused on providing quality, compassionate care. As a Social Worker in our Stevens Point area, you will provide direct social work intervention to patient/family, work with interdisciplinary team to develop and implement coordinated plan of care. If MSW, supervise work of staff BSW'a and/or student interns.  

Responsibilities include:

1)      Provides professional social services by:

·        Responding to referrals and providing services as appropriate.

·        Assessing psychosocial needs of patients and for families and consulting with members of the interdisciplinary Ministry Home Care team.

2)      Serves to plan and implement the medically prescribed Physician's Plan of Treatment for social services and evaluate results.

3)      Enhances the quality of patient care by:

·        Providing advisory, consultative and educational services to patient/family, community and to Ministry Home Care Staff as indicated.

·        Implementing services in accordance with the established care plan and in compliance with third party regulations, physician orders, certified home care requirements, Ministry Home Care policies, procedures, protocols and standard of practice.

·         Use of community resources and coordinating appropriate referrals.

4)      Provides comprehensive documentation in the patient's medical record that is reflective of the patient/family response to interventions.

5)      Identifies actual or potential quality/safety/ethical/legal issues in patient care and participates in resolution strategies.

6)      If MSW, supervises staff BSW's and/or student interns.

Part-time position. 20 hours per week.  Hours will be between 8:00AM and 4:30PM. No Weekends

 

  • Masters of Science in Social Work from an approved school preferred. Will consider bachelor’s degree in social work, psychology, sociology, or other related field AND one year experience for BSW.
  • Minimum of one year experience in hospital, home care, or geriatric social work services preferred.
  • Current license in the State of Wisconsin
  • Valid Wisconsin driver's license, use of car and proof of automobile insurance.
